A recent passport size photograph should be affixed on the application. One additional copy of passport size photograph should also be sent along with the application.The filled application in the prescribed format should be send to Administrative Officer-III, Govt. of India, Dept. of Atomic Energy, Heavy Water Board (Central Office), Vikram Sarabhai Bhavan, 4th floor, Anushaktinagar, Mumbai - 400 094 on or before 30th November 2009
